First Visit Appointment (60 minutes)
Description
This appointment will require you to fill out a health history, including a brief discussion and assessment with your therapist followed by your massage treatment. Please be sure to arrive a few minutes early for this appointment if you did not complete the electronic version of your health history form. Direct billing is available for most insurance companies -please bring your insurance benefit card with you along with a copy of your doctor's referral if required.
